Dr. Cook attended Southern Illinois University where he attained his bachelor's degree, completing his doctor of osteopathic medicine degree at Midwestern University followed by a residency in family medicine at the University of Illinois. Certified by the American Board of Family Medicine, Dr. Cook also holds certification in addictions medicine with the American Society of Addictions Medicine. In addition to his private practice, Dr. Cook serves as an instructor at the University of Illinois, Midwestern University and Rush University. For more than a decade, he has also volunteered at CommunityHealth Chicago, providing top-quality care for lower income patients.
