See What Our Patients Say About This Location:

I have been a patient since moving to Chicago in 2016. This office is providing eligible patients with the Moderna vaccine. This is no small feat for them to get the vaccine. Hats off to Dr. Cook and medical assistant Lupe who made this happen. A totally safe and comfortable experience like every visit to this office. Very caring and totally committed to their patients.

Ater my last doctor moved out of Chicago, I found myself with the daunting task of finding a new doc. Two different friends recommended Dr. Cook. The most difficult part of the transition was actually getting an appointment, but after a few phone calls and emails my first appointment was confirmed. This was just over a year ago and I have found the office staff and Dr. Cook to be professional, friendly, and eager to go the extra mile. Dr. Cook has also recommended me to a specialist who has also been great and is helping me address a chronic condition. The office staff know me and my healthcare needs. I’ve never had to wait more than 5 or so minutes to be seen and Dr. Cook is patient, taking as much time as necessary to fully answer my questions and to provide me the necessary background on my condition. I never feel rushed or that I’m not being taken seriously. His office is convenient for me, about 15 minutes from both home and work. Strongly recommend!

Getting my covid vaccine was the happiest appointment with Dr. Cook and his amazing team. The office is sparkling clean and patient appointments are scheduled so that you do not need to see anyone but the doctor. Before leaving a room, they always make sure the hallways and lobby is clear of people and that you can safely distance on your exit. Even though everyone is masked, you can tell the staff is giving you a warm welcoming smile. Is it possible to have your doctor office to be one of your favorite places in all of Chicago? Dr. Cook just the best. He always is compassionate and caring and listens so well. I always leave with a smile. Thank you Dr. Cook!

First of all I was going to him for over 10 years, he introduced me to a meditation that saved my life. To new patients what I suggest is to be up front and honest with him he is here to help. I moved to kc mo to get a way from Chicago and I am doing better then I have ever done. But if i wouldn't of meet this man i wouldn't be here today i have seen so many people die from this disease. And it brakes my heart because they are getting younger and younger every year. Remember to be responsible with your medication and listen to dr cook he is here to help people like us, and there are not to many dr that are. Please do as he says and go to your meetings, and I promise you if you do all this your life will get so much better and brighter I promise work the program it works. Good things happen when you do the right thing I promise I relapsed over 30 times if I can Do it anybody can. God is good.

When I moved to Chicago Dr Cook became my primary doctor. Today I continue to visit Dr Cook even though I live an hour away in the suburbs. Dr Cook is easy to talk to and very caring for his patients.
